Licence Amendment
How to amend your licence
Under Irish legislation (S.I. No. 125 of 2000), licensees must obtain the prior consent of the RPII before modifying their practices, procedures equipment or facilities.
Changes to a licensed practice which require an amendment include:
- A change to the licensee’s name or address
- The addition or removal of a licensed item
- The addition or removal of a licensed practice (e.g. disposal, transportation etc)
- The addition or removal of a premise
- The addition, removal or change of an appointed Radiation Protection Adviser
- The addition, removal or change of an appointed Radiation Protection Officer
- A change to the status of a licensed item (e.g. change of a new item from use for commissioning purposes to full use or the withdrawal of an item from use).
A licensee may request changes to its licence by completing the appropriate licence amendment form which can be downloaded (see below). The amendment must be supported by documentation mentioned in the form.
For example, when purchasing a new radiation source, or changing a work practice the following information must accompany the completed amendment form
- A new or revised risk assessment
- Revised radiation safety procedures.
Downloadable forms
If you wish to make changes to Schedule 2 (Inventory of licensed items) or Schedule 3 (Radiation Safety Committee/Radiation Protection Adviser/Radiation Protection Officer/ Industrial Radiographers) of your licence, you must complete and forward the appropriate licence amendment form. Changes may not be implemented until authorisation has been received from the RPII.
